HIPAA Compliance: This Health Benefits Review is conducted through SurveyMonkey, a platform that has executed a Business Associate Agreement (BAA) with United and Guided. This means that SurveyMonkey is legally bound to handle your PHI in compliance with the Health Insurance Portability and Accountability Act (HIPAA).

Data Security: All responses are encrypted in transit and at rest. Only authorized personnel at United and Guided will have access to aggregated results for program planning and evaluation.

No Individual Tracking: Your individual responses will never be sold, shared, or used for purposes outside of community health planning and improvement.
Legal Disclosure: By completing this survey, you acknowledge that your information will be processed and stored in accordance with HIPAA requirements under the BAA between United and Guided and SurveyMonkey. Your PHI is safeguarded under federal privacy and security regulations.
