Section D focuses on Social Determinants of Health, which are the social and economic factors that affect your overall health. This includes things like access to stable housing, food, transportation, and employment. These factors can be just as important as medical care when it comes to your well-being. A client we worked with had struggled with housing instability for years, which kept them from focusing on their health. After identifying these social factors, we were able to connect them to housing supports and additional resources, which significantly improved their ability to manage their health conditions.
