A personalized care plan is your roadmap to whole‐person wellness. Structuring one ensures that all aspects of your health—physical, mental, and family—are identified, supported, and connected. It allows public health organizations like United And Guided to tailor services so that they align with your unique needs: medical history, emotional well‐being, social circumstances, and family responsibilities. Without this structure, gaps often emerge—maybe you have clinical care but lack support for mental health, or you manage a chronic condition but can’t access transportation or stable housing. These gaps tie directly into your social determinants of health: the conditions in which you are born, grow, live, work, and age (including economic stability; education; neighborhood & physical environment; healthcare access; and social & community context). When these determinants are thriving, your health outcomes improve, and you can lead a more fulfilling life. United And Guided offers a FREE, personalized care plan if you complete a comprehensive needs assessment—broken into Sections A through F. By going through each section, you help ensure your physical, emotional, and family health care needs are all met—and that your social determinants are addressed. This plan empowers you, letting healthcare resources work with your real life—not apart from it.

Section A: Focuses on initiating the care planning journey. You meet with Lead Care Managers to discuss your personal goals, challenges, and medical needs. This section builds trust, helps assess eligibility for Medicaid benefits, and introduces how your life experiences (like Self-Care Sundays) shape the support you need today.

Section B: Covers your medical history and current health status. It helps identify past diagnoses, treatments, and ongoing health conditions, allowing the care team to design a plan that fits your unique health journey—not just generic checklists.
