What This Service Is

Rather than focusing on one narrow goal, a Day Habilitation appointment starts with a conversation about the member's own priorities, and builds a plan around them.

United And Guided provides Day Habilitation as a nonclinical, community-based service. It does not replace vocational rehabilitation, clinical assessment, or licensed treatment services; when specialized support is needed, the team may help connect members with appropriately qualified providers.

  6. Follow-up and progress review. The team checks in to review progress and adjust the plan as needed.

Managed care plans, county agencies, schools, correctional and reentry partners, employers, and community-based organizations may refer members who could benefit from Day Habilitation, and may coordinate directly with United And Guided's team on shared members.

Providers may submit a standard referral through the Refer a Member form, or coordinate a warm handoff — a direct, real-time introduction between the referring partner, the member, and the United And Guided team — when a more immediate connection would better support the member's transition into service.
